We participate in many managed care plans. To insure that we are listed as an in-network provider for the plan you've selected, please call the phone number listed on your insurance card for confirmation. Should your insurance plan require a referral, it is your responsibility to obtain that referral from the primary care provider listed on file with your plan prior to your visit.

The insurance representative will be able to tell you about covered and non-covered services as well as co-payments, deductibles and co-insurances affiliated with your chosen insurance plan. Please be prepared to pay any co-payments and deductibles at the time of service as this is another requirement by your insurance company.
